What you share

The form asks for your name, date of birth, number of people needing coverage, coverage interest, ZIP code, phone number, email address, and whether there are health issues to discuss. You can choose “Prefer to discuss” for the health question. We also ask for permission to share these details and contact you about your request.

The form does not ask for medical records, diagnoses, Social Security numbers, or payment details. Please do not add that information to any field.

How the inquiry is used

Your answers are used by Iron Standard Wellness to respond to your inquiry and discuss your coverage needs. Submitting the form does not enroll you in coverage or subscribe you to a marketing newsletter.

How the request is processed

Formspree processes submitted form answers and makes them available to Iron Standard Wellness through its configured submission and notification services. The website hosting service also processes web requests and technical information needed to operate the site.

Your answers remain in this page while you complete the form. This website does not save them in browser storage or include them in page URLs or application logs. They are submitted when you select “Send my request.”
